Job Description

Assesses, determines, and coordinates placement of all BMH admissions and transfers in accordance with clinical standards and guidelines. Ensures patients are placed in clinically appropriate level of care via established algorithms and protocols. Upholds standards of system wide customer service program.

Experience

5 years of experience in nursing with 2 years of experience in critical care or emergency nursing with advanced clinical assessment skills

Preferred Qualifications

Bachelor of Science in NursingExperience in progressive leadership and training as house supervisor or bed placement coordinatorExperience in psychiatric or behavioral healthExperience in a medical inpatient/emergency department setting working with pediatric and adolescent patient populationsExperience with behavioral mental health (BMH) facilities and placement procedures

Education

Graduation from an accredited school of nursing

Licensure as a Registered Nurse in the single State of Georgia or Multi-State through the Enhanced Nurse Licensure CompactCertified Case Manager (CCM) or Psychiatric Mental Health (PMH-BC) Certification (current or within 2 years of hire)Basic Life Support (BLS) within 30 days of employment

Job Responsibilities

Performs the nursing process while providing indirect patient care in a safe and effective manner that promotes quality patient outcomes. Functions as member of interdisciplinary healthcare team.Performs an initial discharge planning evaluation or Emergency Department assessment for patients.Provides supervision of Licensed Practice Nurse Navigator activitiesCultivates and maintains effective interaction/communication with medical staff, nursing staff, social workers, and others to drive the care coordination process and facilitate continuity of patient care.Communicates with home health agencies, third-party payor, and other community resources as needed to coordinate needs.Facilitates clinical practice guidelines at the patient level.Performs outpatient and clinic care coordination and monitors patients' care amongst the hospital, clinic, and home.Facilitates care via telehealth to assist with patient needs and concerns.Provides health promotion activities and education for caregivers and staff at assigned facility, including workshops and trainings.Refers cases identified as risk management issues, peer review issues, or quality issues to appropriate personnel.Coordinates patient placement activities to optimize patient flow, patient outcomes, admission appropriateness, and cost efficiencies in support of organizational strategic initiatives while working with campus house supervisors, physicians, and nursing staff.Communicates and collaborates with leaders and staff at external organizations/agencies regarding placement needs for whole childcare.Maintains assigned databases.Collaborates in collection of data for identification and resolution of flow issues.Collects, analyzes, and interprets outcome data systematically to identify problem areas and assess need for improvement and/or support changes in system-wide processes in relations with the Behavioral Mental Health strategy.Prepares periodic reports as directed.Participates actively and contributes to organizational initiatives that impact patient flow.Supports continuous performance and quality improvement.Communicates openly with appropriate leadership team to ensure knowledge of key decision factors.Serves as expert clinical resource for patient placement by ensuring smooth functioning of the patient flow process to reduce wait times and avoid unnecessary patient movements.Maintains positive working relationship and demonstrates excellent customer services skills in all interactions between amongst the interdisciplinary team, physicians, internal customers, and external customers.
