Bereavement Coordinator Flexi
Summary The Bereavement Coordinator is responsible for maintenance and coordination of care for the patient and family experiencing perinatal loss while under the guidance and direction of the Nurse Manager. The Bereavement Coordinator practices within the guidelines of the Nursing Care standards and hospital policies and procedures and manages required staff training and education in conjunction with the assigned Nurse Educator. Additionally, the Bereavement Coordinator coordinates across disciplines for the support and follow up of care with families who have experienced loss and serves as the primary point of contact for community outreach services addressing perinatal loss. Essential Duties and Responsibilities The duties and responsibilities described below represent the general tasks performed on a daily basis. Other tasks may be assigned. + Coordinates care of families with Perinatal Loss: + Follow-up with all families who have experienced loss + Sending card to all families with messages from staff who cared for the patient + Follow-up phone call to families with assessment of: + Has the patient scheduled a follow-up appointment with OB Provider? + Does she have the support of Family/Friends? + Is she interested in a support group? + Provide patient educational material to surgery center for patients who have experienced miscarriage. + Liaisons with Rachel’s Gift in coordinating the annual Chesapeake Regional Healthcare sponsorship of Angel Dash Walk (requires Nursing leadership, Fiscal and Marketing support) + In conjunction with the Chaplain, coordinates the annual Chesapeake Regional Healthcare Memorial Service + Collaborative professional relations with Chaplains, Nursing Supervisors, staff on Labor and Delivery and organizations that support the Bereavement Program + Provides training and education to all Labor and Delivery staff in caring for Perinatal loss to ensure staff: + Offer Chaplain services for blessing and prayer + Work with the family to ensure there are no regrets: + Ensure mother and father have the opportunity to hold the baby + Ensure pictures and footprints, lock of hair, etc., are obtained as mementos + Work with family in end-of-life decisions; cremation, burial, selection of funeral home, etc. + Provides expertise and research in policy guidance as needed for the development of policies specific to perinatal loss and care. + Facilitate organization, maintenance and functioning of equipment and the Bereavement Room: + Ensure adequate supplies and clothing + Maintain “Cuddle Cot” supplies and training of staff + Maintain camera and printer supplies and functions + Order all Bereavement supplies Qualifications To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Education and Experience Minimum Required Education: Bachelor’s degree Preferred Education: BSN Experience: Significant related experience (3-5 years), Perinatal Loss Training completion (within 3 months of hire) Certificates, Licenses, Registrations Preferred but not required
