Offers may very depending on the circumstances of each case. **Summary:** The Bereavement Counselor LCSW will be a core member of Cook County Health Palliative Care Interdisciplinary Team and will report to the Section Chief of Palliative Medicine in the Division of General Internal Medicine. This position will provide bereavement services for patients and families served by CCHHS and bereavement support to the interdisciplinary team and hospital personnel, as needed. The Bereavement Counselor provides on-site and follow-up (both in person and via telephone) clinical interventions/consultations for patients and families, including children, around grief and loss. The individual who holds this position exemplifies the Rush mission, vision, and values and acts in accordance with Rush policies and procedures. **Other information:** **Required Job Qualifications:** • Master’s Degree in Social Work required. • Current State of Illinois licensure as a Licensed Clinical Social Worker (LCSW) required. • At least three years of related experience in bereavement counseling and health care required. • Excellent interpersonal skills. • Strong written and verbal communication skills. • Proficiency with the Microsoft Office Suite. **Preferred Job Qualifications:** **Responsibilities:** • Provides clinical assessment, counseling, and support to the bereaved and oversee the development of bereavement services through follow-up to families after patients die. • Provides clinical assessment, counseling, and support to bereaved parents and families after perinatal loss and oversee the development of bereavement services through follow-up. • Provides telephone contact with bereaved family members of patients to introduce services and offers intervention. • Facilitates bereavement support groups on-site and provide individual support strategies for families, including children, members of the interdisciplinary team, or hospital personnel. • Accepts referrals and provides counseling services to bereaved individuals and families throughout the community served by CCHHS, including home visits as needed or desired. • Demonstrates and applies knowledge of emotional, social, and spiritual needs and responses to death and grief. • Oversees the development and sending of periodic follow-up letters to family about grief. • Participates in the orientation, education, and training of new staff, rotating physicians, nurses, social workers, and other health care professionals, including psychology or social work interns. • Participates in providing support to hospital staff related to death, loss, and job-related stress, including coordination of the annual memorial service for families and staff • Facilitates linkage to community services for bereavement as desired by patients and families. • Demonstrates and applies knowledge of diverse cultural and spiritual values and customs related to illness, death. and mourning. • Maintains quality and productivity standards and participate in the measurement of these standards.
