Bilingual Centering Pregnancy Educator

Schedule

Full Time: 40-hour work week with centering sessions occurring in the evening 3 - 4 times per month, 6:00 pm - 8:00 pm.

General Summary

Centering Pregnancy education is the primary responsibility of this position. It is the expectation that all Centering Pregnancy meetings will be attended, and this may primarily occur in the evening hours. Under the general supervision of the Clinical Nursing Coordinator, the Practice Manager, or Physician/APP, works as a member of the patient care delivery team, and as designated per established policies and procedures, assumes primary responsibilities for technical, multi-skilled direct and indirect patient care activities. Establishes trusting relationships with identified disparate population groups that strengthens the patient-provider relationship and improves patient care. In addition, disseminates health information in a culturally relevant manner and builds individual and community capacity to address health care needs.

Duties and Responsibilities

Essential Functions:

Communicates information about health, health education and healthcare needs, concerns and resources to appropriate audiences.Assesses potential areas for health education programs/services and identifies resources available to meet the needs of the community.Evaluates health promotion/education programs, communicates the results of the evaluation and makes recommendations.Implements or coordinates the implementation of health promotion/education programs.Networks with community health organizations to effectively address health education issues and improve resource utilization.Assists with program marketing and publicity for activities.Assesses needs of population, patient or individual on an ongoing basis. Identifies barriers to care and makes appropriate recommendations for follow-up or referral.Educates community members, either individually or in groups, about how to access and/or use the health care system. Shares relevant concerns from community members with medical providers or others within the health care system.Identifies and utilizes culturally appropriate outreach strategies to provide specific health promotion and disease prevention education to specific population groups.Evaluates program outcomes/service effectiveness through tracking participation levels, conducting participant surveys, reviewing chart records, and prepares the quarterly and yearly state reports as needed.Actively participates on community coalitions and boards as an advocate for specific populations and as a liaison for WellSpan HealthDocuments and maintains accurate and confidential records of client interventions.Obtains and documents reason for visit, patient vital signs, including temperature, blood pressure, pulse, respirations, pulse oximetry, height and weight.Prepares patient for visit and/or procedure by assuring the patient is undressed and appropriately gowned if appropriate.

Qualifications

Minimum Education:

Associates Degree Required

Work Experience:

2 years Experience as an educator in a health-related field/organization. Required

Licenses:

Basic Life Support Upon Hire RequiredCertified Nurse Aide Upon Hire Preferred

Courses and Training:

Complete Centering Basic Facilitation workshop (employer provided). within 180 days Required

Knowledge, Skills, and Abilities:
