Bilingual Outreach Case Manager
Downtown Streets Team (DST) is a nonprofit organization committed to fighting homelessness through the power of work and dignity of community. DST increases social capital and economic mobility by helping our unhoused neighbors chart a way out of homelessness through community building, volunteering opportunities, employment and human-centered case management. Since 2005, DST has expanded across 16 cities in Northern and Central California, serving over 1,500 people annually. SUMMARY: The Outreach Case Manager will connect with individuals experiencing homelessness in encampments, RVs and one-off individuals residing on Binford Rd, engage with them, assess them for Coordinated Entry, and help them reduce barriers to exiting homelessness through strong organizational, problem-solving, and analytical skills, while managing priorities and workflow. Hours for the position are Tuesday through Saturday, 12PM to 8PM. The Housing Case Manager will need to be able to communicate fluently in Spanish and English. WHAT YOU'LL DO: Educate and roll out best practices for homeless outreach, street-based case management and de-escalation Practice outreach safety best practices in the field at all times Coordinate and distribute outreach supplies to individuals in encampments and RVs and on Binford Rd Conduct VISPDATS and other vulnerability assessments to individuals residing on Binford Rd Accurately and promptly document all interactions (phone, meetings, etc.) via case notes with all individuals encourtered during outreach Enroll individuals connected with during outreach into HMIS Constantly update Tracker, Marin HMIS, Oasis, and Umbrella for accurate reporting Tracking impact and reporting in alignment with County contract Research services outside of the agency that may aid individuals in their goals of exiting homelessness (i.e. St. Vincent de Paul, Ritter, etc.) Attend all weekly, biweekly and monthly encampment meetings throughout County Provide compassionate support and assistance for individuals in crisis Screen individuals for Covid-19 and work with partners on testing and quarantining Utilize any available subsidies and affordable housing units to place participants into housing Establish productive relationships with each and every participant with no bias or prejudice towards race, religion, age, sex, disability, or sexual preference Must maintain and adhere to written confidentiality protocols. COMMUNITY SPECIFIC RESPONSIBILITIES: Build rapport and trust with outreach recipients through trauma-informed and client-centered outreach. This will include addressing their immediate prioritized needs, such as transportation, support, communication (phones), food, emergency supplies, tent repair, etc., which will open the door for residents to eventually accept the offer of moving into permanent housing. Encourage but not mandate participation in services. The team shall engage participants by building trust and rapport and embracing a nonjudgmental approach. Maintain a predictable schedule to build trust with outreach recipients Provide Coordinated Entry assessments when appropriate Support outreach recipients to acquire documentation necessary to enter housing if/when housing becomes available (e.g. photo ID, birth certificate, etc.) Connect outreach recipients to resources for which they may be eligible, including but not limited to veterans benefits, medical care, employment assistance, credit repair, emergency shelter or other housing supports, etc. Offer problem-solving services to identify any and all possible housing options that may be available, even on a short-term basis. Support recipients to manage their personal belongings while unsheltered, including helping them understand any local encampment restrictions, and supporting them with sorting through and disposing of items that they no longer want. Not require sobriety to receive housing or services, and no drug testing shall be conducted. Instead, case managers shall rely upon motivational interviewing and harm reduction philosophies to help participants minimize the harm associated with substance use and define a realistic and personalized pathway to recovery. Consistently collaborate and problem solve with second Outreach Case Manager and Housing Case Manager to ensure clear pathways to housing Distribute socks, masks, hand sanitizers and hygiene kits during outreach Advocate for individuals living in vehicles to be connected to the Rapid Rehousing (RRH) pathway. Report as required to all city and county partners Hours for the position are Tuesday through Saturday, 12PM to 8PM, often operating without direct supervision in the field. WHAT YOU'LL NEED: HS Diploma or equivalent Ability to perform job duties with minimal supervision Excellent listening comprehension, written and verbal communication skills Experience in Microsoft programs Fluent in Spanish and English PHYSICAL REQUIREMENTS: While performing the essential duties of this job, the employee is frequently required to do the following: Sitting, standing, walking, bending, stooping, pushing, pulling and lifting (up to 25 lbs.) Using personal computer, keyboard, mouse and monitor Observation and problem solving; hearing, seeing, reasoning and decision making Basic mathematical ability WORK ENVIRONMENT: Moderate noise associated with an open office work environment. Work environment and physical location may vary, depending on the space available and the needs of the community and the program.
