families and staff to ensure exemplary service providing efficient timely and coordinated guidance and support throughout the care continuum screening diagnosis treatment and supporteducation services The Breast Health Patient Navigator partners with patients and providers to offer a personalized individualized approach to womenrsquos health care including the screening and diagnosis of malignant and benign breast disease Responsibilities will include but will not be limited tomiddot Provides patient education and support on pre and post procedure instructions for Stereotactic biopsy Ultrasound guided biopsy and MRI guided Breast Biopsy reinforcing information about diagnosis treatment plans and available resourcesmiddot Initiates communication with patients that are given the recommendation for a breast biopsy Assistdirect the patients with scheduling and screening for the biopsy Assist with any questions andor concerns Confirm with referring offices that patient abnormal biopsy results were receivedmiddot Communicate and collaborate with the patientrsquos referring provider to improve timeliness of care and patient satisfactionmiddot Create and maintain statistical analysis programs across all breast imaging modalitiesmiddot Work in collaboration with the imaging department to assist in preparing case presentations for all cancer conferences related to
