The Ohio Department of Health is undertaking the Secretary's Minority AIDS Initiative Funding for Care and Prevention in the United States (CAPUS) Demonstration Project
Complete Description:
The purpose of the grant is to reduce HIV related morbidity, mortality, and related health disparities among racial and ethnic minorities in the United States. Grantees are required to 1) increase the proportion of racial and ethnic minorities with HIV, who have diagnosed infection, by expanding and improving HIV testing capacity and 2) optimize linkage to, retention in, and reengagement with care and prevention services for newly diagnosed and previously diagnosed racial and ethnic minorities with HIV by addressing social, economic, clinical, and structural factors influencing HIV health outcomes.
One of the grant required activities is to use laboratory surveillance data to indicate when persons living with HIV are receiving care, not receiving care, or have lab data that indicate poor clinical outcome. Local health department disease intervention specialists (DIS) and/or linkage to care coordinators (LCC) will contact the patient, health care provider, or both to re-establish engagement in care, improve services, and overcome structural, social, or economic obstacles to achieving desired health outcomes (e.g. viral suppression).

4 year college degree or equivalent.
Role Description:
1) All roles specified plus additional work experience and job knowledge.
2) Serves as the liaison between Program customers and the developers.
3) Cover high level scope and objectives with the Program customers of the project and establish communication channels.
4) Collecting and documenting both detailed business and functional requirements for the purpose areas identified above. The functional requirements could also include
***documenting the data mapping between two distinct applications and data models,
***data reconciliation method,
***data mediation between a data source and a destination,
***(identification of data relationships as part of data lineage analysis (documenting where the data is and how it flows) and
***consolidation of multiple databases into a single data base and identifying redundant columns of data for consolidation.
5) Create system and business process work ups and application prototypes.
6) Work directly with technical lead in assigning tasks and time estimates.
7) Have regular interactions with the developers to clearly transfer knowledge of the user expectations.
8) Play a major role in the testing phases by
***ensuring the coded modules are fulfilling the requirements,
***providing test scripts and
***support during the user acceptance testing phase.
