Job Summary:

Responsible for planning, directing, and evaluating a comprehensive activity schedule of meaningful activities that reflects resident preferences for social, intellectual, physical, cultural, spiritual and political desires. Provides group and individual activities, interviews resident, and meets New York State Department of Health documentation standards.

Essential Functions:

Plans, directs and evaluates activity schedule to ensure activities reflect resident preferences for social, intellectual, physical, cultural, spiritual, and political desires.

Interviews residents upon admission and quarterly

Documents resident interviews, activity attendance, and generates quarterly activity notes

Provides group and individual activities

Participates in person-centered service plan meetings

Participates in policy and procedure formation and performance improvement activities

Attends Meadowbrook management meetings

Develops and maintains relationships with outside community organizations

Supervises Activity Assistants

Qualifications

Education:

Associate Degree in Recreation OR 2 years full time experience working in recreation with a dependent adult population

Experience:

Two years full-time experience in the recreation field with a dependent, adult population required
