Cardiac Rehab Coordinator
**It's more than a career, it's a calling** WI-SSM Health St. Clare Hospital - Baraboo **Worker Type:** Regular **Job Highlights:** + **_Monday through Thursday from 6:00AM-4:00PM_** + **_May be adding in Fridays from 6 :00AM– 1:00PM or 2:00PM_** + **_40 hour/week position_** + **_Outpatient Services_** + **_Working knowledge of Phase II and Phase III Cardiac and Pulmonary Rehab._** + **_The candidate may need to work the floor as well as perform coordinator duties._** + **_Duties include coordination of staffing needs, coordinating the certification application for AACVPR, and guiding growth of the department._** + **_Knowledge of EPIC and Scottcare is a plus_** **Job Summary:** Provides direct patient care to cardiac and pulmonary rehab patients including various types of monitoring, education and therapeutic exercise programs. **Job Responsibilities and Requirements:** PRIMARY RESPONSIBILITIES + Assesses patient needs daily through chart review, unit nurses or cardiac nurse, physicians and interdisciplinary team. + Provides exercise prescription for patients utilizing test data, patient status and current medication regimen; updates prescription according to the patient's response. + Schedules and coordinates patient assessment, testing, results reporting, treatment and follow up as needed. + Assesses hemodynamic response and patient tolerance to activity progression and utilizes information to provide appropriate exercise prescription. + Performs appropriate documentation of monitored ambulation, education and discharge planning per departmental policies, guidelines and practices as pertinent for setting. + Participates in routine maintenance and calibration of the test equipment to ensure its proper operating condition and reporting the need for major repair. + Assists in recruiting/interviewing and onboarding of new associates. + Creates quality improvement monitoring strategies and improvement initiatives. + Assist in monitoring of staff performance. + Provides exercise information for patients, peers and the community which may include public speaking and education classes. + Works in a constant state of alertness and safe manner. + Performs other duties as assigned. EDUCATION + Bachelor's degree EXPERIENCE + No experience required PHYSICAL REQUIREMENTS + Frequent lifting/carrying and pushing/pulling objects weighing 0-25 lbs. + Frequent sitting, standing, walking, reaching and repetitive foot/leg and hand/arm movements. + Frequent use of vision and depth perception for distances near (20 inches or less) and far (20 feet or more) and to identify and distinguish colors. + Frequent use of hearing and speech to share information through oral communication. Ability to hear alarms, malfunctioning machinery, etc. + Frequent keyboard use/data entry. + Occasional bending, stooping, kneeling, squatting, twisting and gripping. + Occasional lifting/carrying and pushing/pulling objects weighing 25-50 lbs. + Rare climbing.
