All while understanding that **together** we are even better. **Schedule:** Per Diem **Shift** : Days **Location** : 5101 Willow Springs Road, La Grange, IL 60525 **The role you’ll contribute:** The Registered Professional Nurse (PRN) is responsible for developing and implementing cardiac rehabilitation treatment plans and provision of cardiac education to adult and geriatric patients and their families. The RN will facilitate delivery of care for the assigned group of patients within this unit, and perform all related clinical activities. **The value you’ll bring to the team:** + Assessment – Collects patient health data + Diagnosis – Analyzes the assessment data in determining diagnoses. + Outcome Identification – Identifies expected outcomes individualized to the patient. + Planning – Develops a plan of care that prescribes interventions to attain expected outcomes. + Implementation – Implements the interventions identified in the plan of care. + Evaluations – Evaluates the patient's progress toward attainment of outcomes. + Assists with the implements of policies, standards, educational services, and programs that support the department. + Demonstrates initiative in participation with quality improvement activities and outcomes of the department. + Participates in activities to enhance revenue and support cost reduction and containment activities. + Shows evidence of productivity and assists with cost center budget compliance. + Assists with compliance of accrediting and regulatory agency standards. + Identifies and correct environmental and practice safety issues. + Resolves patient satisfaction concerns in a timely manner, assuring they have been thoroughly addressed and appropriate actions taken. + Submits reports and requests for information by established deadlines. + Represents UCM AdventHealth Hospital in a professional manner to employees and community members. + Promotes professionalism in appearance, positive attitude, and confidentiality. + Functions in all appropriate roles in the Cardiac Rehabilitation Services program. + The RN practices in accordance with the established Federal and Illinois laws. + The RN is responsible for maintaining general mandatories established by UCM AdventHealth Hospitals and any unit-specific mandatories, as deemed necessary for practice within that setting. + The RN is responsible for legal, ethical practice; serves as a patient advocate in accordance with the patient bill of rights; and is responsible for compliance with regulatory standards pertinent to cardiac rehabilitation practice. + Completes other related duties as assigned.
