Job Summary
The Cardiogenic Shock/ Venous Thromboembolism (VTE) Patient Care Coordinator is responsible for planning, coordinating, and evaluating the care given to patients with cardiogenic shock and/or patients with VTE. This position requires skill in interpersonal relations, problem solving, clinical practice, and teaching.
Responsibilities
1. Resource Management:
a. Utilizes communication strategies and promotes problem resolution between the members of the multidisciplinary team.
b. Evaluates effectiveness and safety of cardiogenic shock and pulmonary embolism response (PERT) initiative policies, procedures, and protocols.
c. Serves as consultant/resource in clinical area of expertise.
d. Identifies problems with support services and follows through to resolve problems.
e. Reviews equipment and supply status monthly.
1. Professional Development:
a. Demonstrates the ability to lead and direct others.
b. Participates in the development of key goals and objectives.
c. Assesses the educational needs of the nursing staff when it comes to cardiogenic shock and to venous thromboembolism. Facilitates educational opportunities to meet their learning needs.
d. Integrates current knowledge of nursing management theories, concepts, and research findings into the practice environment.
e. Collaborates with the Cardiogenic Shock Initiative Medical Directors in research projects relative to the use of mechanical circulatory support.
f. Facilitate education needs of the nurses.
g. Keep call schedule and ensures that call is covered between cardiogenic shock initiative and ECMO coordinator.
h. Collect pertinent data, organizes data and reports data to selected data banks and research projects for both the cardiogenic shock team and the PERT.
i. Assists in the review and collection of data to aid in critical assessments of cardiogenic shock/PERT initiative procedures and practices.
j. Establishes goals for own professional growth.
1. Customer Service:
a. Identifies problems with quality of patient care and initiates actions for resolution.
b. Develops and evaluates educational materials for patients/families.
2. Quality Patient Care:
a. Establishes standards of practice specific to cardiogenic shock initiative in collaboration with the Cardiogenic Shock Initiative Medical Directors.
b. Collects and distributes current cardiogenic shock and VTE related literature to bedside nursing staff.
c. Plans and coordinates cardiogenic shock initiative and PERT meetings including documentation of their content. Includes patient reviews and registry updates.
d. Documents information related to risk management.

Education
• Bachelor's Degree (Required) Experience: A Critical Care Nurse with a minimum of 2 years of experience required. Minimum of 3-5 years of experience as a Registered Nurse, 2 years in an ICU setting. Bachelor of Science in Nursing preferred.
