Mayo Clinic's cardiovascular surgery (CVS) practice provides comprehensive surgical and mechanical circulatory support care for patients with a wide range of illnesses. The CVS practice performs 2000-3000 operations per year for complex surgical or cardiothoracic transplant patients. The thoracic transplant program has experienced unprecedented growth, offering solitary, dual, or triple organ options to highly sensitized or otherwise extraordinary cases. Both CVS and Transplant programs are academically focused on teaching, quality improvement, and high-quality scientific research. This pharmacist will serve alongside exceptionally talented staff and receive support from the Department of Pharmacy, Cardiovascular Surgery and Transplant Leadership, and Mayo Clinic. Our Department of Pharmacy has a dedicated ambition for creating a patient-centered, integrated care model. In stride with these goals, in 2023 alone, our department reached over 200 scientific publication outputs and over 80 quality improvement projects. Over 250 pharmacy staff attained academic rank in the Mayo Clinic College of Medicine and are being increasingly recognized on regional, national, and international stages. Our department’s pharmacy residency has been accredited by ASHP since 1973. It has grown to become the primary training site for 21 pharmacy residents across multiple specialties and learners from around the country. Additionally, our staff regularly interface with and educate medical trainees including physician residents and fellows. Daily pharmacist clinical services consist of robust bedside rounding as part of a multidisciplinary team, identification and intervention of drug-related issues, recommendations on optimization of patient-specific drug therapy, admission and discharge medication reconciliation, transition of care planning, routine use of expansive collaborative practice agreements, response to medical emergencies including cardiac arrest and mass casualty events, and direct patient care on a rotating basis. Involvement in scholarly activities including quality initiatives and multidisciplinary research is encouraged and supported through extensive institutional and departmental resources. As an institution valuing education, preceptorship and mentorship of learners is expected. An example 4-week schedule consists of dayshifts in CVS or thoracic transplant for 2-3 weeks, 3-4 evening shifts preceding every fourth weekend coverage, and two 12-hour mixed medical/surgical ICU weekend shifts every 4th weekend surrounded by three weekday compensatory days off. Provides medication management services in coordination with other health care professionals to deliver safe, appropriate, and cost-effective medication therapy to patients. Responsibilities may include assessment of medication therapy regimens, medication therapy management, medication order verification/entry, medication distribution, medication therapy monitoring, and consultation, documentation of medication therapy plans, and medication use education and pharmaceutical care services. Participates in collaborative practice and/or protocol-driven practice as appropriate. Additionally, pharmacists are active members in quality improvement, research activities, and education as described in the job specific competencies. **Qualifications** Qualifications + Graduate of an ACPE-accredited Doctor of Pharmacy (Pharm.D.) program or Bachelor of Science (B.S.) with a minimum of 2 years relevant experience. Preferred Qualifications + Expertise in CVS and thoracic transplant critical care and step-down unit cares will be needed in this role. Preparedness may come as a combination of formal training and work experiences, e.g., completion of a PGY2 residency or fellowship training in critical care, cardiology, or solid organ transplant; PGY1/non-transplant PGY2 residency plus two years of experience in critical care, cardiology, or thoracic transplant practice; or minimum of five years of critical care, cardiology, or thoracic transplant experience. + Evidence of ongoing scholarly pursuits including research/quality improvement, publications, or scientific presentations. + Relevant applicable pharmacy experience working in a progressive and collaborative practice environment. + Experience and/or interest in teaching and/or precepting. + Commitment to excellence, accuracy, attention to detail and teamwork required. Ability to communicate effectively, work collaboratively with others, organizes time well, practice with an agile mindset, solve problems, and work independently with minimal supervision required. License or Certification + Current pharmacist license by the MN Board of Pharmacy, or eligible to obtain within a time frame compliant with the site’s applicable employee licensure policy.
