Position Overview:
Performs a wide range of clerical duties and a variety of office functions in support of coordination of communication among members of the Interdisciplinary Team (IDT), participants, and outside providers. Serves as primary point of contact between participants, caregivers, and care team. Schedules inside and outside doctors' appointments.Assures inter and intra departmental communications to coordinate participant care via verbal, written, and email communications.Reviews physician’s orders to assure timely and accurate scheduling of testing and procedures. Maintains complete and accurate participant schedules, including the amount, frequency, and duration of service that has been identified in the plan of care.Notifies appropriate manager when requests for scheduling changes are received that require a physician's order. Records all scheduling changes, updates, and deletions on the schedule daily. Responds promptly to participant referrals, oversee service delivery, and monitor participant satisfaction.Effectively and accurately coordinates and communicates with all departments within Capital Health PACE that are involved with the participant appointments.Works closely with clinical leaders to ensure accuracy of all information and to ensure accuracy of all information and to ensure that participant education has been completed by nursing prior to appointments.Coordinates home health aide services for appointments as determined by participant care plan. Creates EMR authorization, transportation appointment, and escort appointment. Calls participants and caregivers and reminds them of the next day appointment(s). Schedules appointments for consulting physicians.Performs accurate and timely scanning of medical record documents. Gathers medical records information needed for physician office visits and uploads provider information returned post appointment into electronic medical record. Obtains and inputs authorizations for services into the electronic medical record in a timely manner.Utilizes excellent telephone etiquette to provide follow-up phone calls to participants regarding appointment scheduling time and revisions. Organizes appointment schedules with transportation department and notifies participants of transportation times.Attends IDT meeting daily and huddles as scheduled to remain current on participant care and appointment needs.Performs other duties as assigned.MINIMUM REQUIREMENTSEducation: High school diploma or GED. One year of college or technical school preferred.Experience: One year of related healthcare experience.Other Credentials:Knowledge and Skills: Ability to communicate clearly, both orally and in writing. Ability to carry out both oral and written instructions. Knowledge and usage of correct grammar, spelling, and punctuation. Competence in basic office practices and procedures such a s filing, document preparation. Excellent organizational and time management skills.Special Training: Expertise with Microsoft Office. Ability to type 60 words per minute. Ability to accurately and efficiently create reports and graphics using Microsoft Excel or other computer applications.
