The role you’ll contribute:

The Care Connector is responsible for assisting in coordination of care and services for assigned patients under the direction of Care Management Leadership, and in collaboration with the multidisciplinary team and patient/family. Interacts with representatives from physician offices, insurance companies, managed care organizations, post-acute providers, and transportation agencies as required. The Care Connector establishes a general working knowledge of payor-specific documentation, regulatory and authorization requirements, the clinical review process, provision of acute and post-acute services, as appropriate for the role. Actively participates in outstanding customer service, escalates as appropriate to effectively and promptly resolve issues, and accepts responsibility in maintaining relationships that are equally respectful to all.

The Care Connector on the post-acute team ensures discharge arrangements are compliant with payor requirements and regulatory guidelines. Responsible for assisting in patient discharge plans as directed, assisting with coordination of clinical data collection, and ensuring third-party payer notifications and authorizations are secured and documented as a support function for Care Management staff.

The Care Connector on the acute care and/or ED team establishes a general working knowledge of post-acute appointment needs. Ensures regulatory requirements are compliant with CMS and AHCA regulations and post-acute appointments are secured prior to patient’s discharge, as appropriate within their role.

Documents activities and progress in the electronic medical record per defined policy and procedures, computer billing/utilization systems, or in other ways as directed. Inputs clinical information and discharge planning details into the medical record as appropriate for the role.

Keeps RN Care Manager, Social Worker and/or other members of clinical team informed of issues or delays impacting a timely discharge or scheduling of post-acute appointments.

Qualifications

The expertise and experiences you’ll need to succeed:

Required:

High School Diploma or GED

At least one-year experience in a customer service environment

At least 6 months of experience in a healthcare environment

Preferred:

Associate degree in a healthcare-related field

Experience working in a healthcare setting
