Care Coordination Lead at Urgent Team
Urgent Team is looking for a Care Coordination Lead to join our corporate office in Nashville, TN (Green Hills).  This is a hybrid role and may require working in the office or a local center occasionally.     SUMMARY:  The Care Coordination Lead is a vital member of the healthcare team responsible for providing comprehensive support and guidance to patients throughout their healthcare journey. This role focuses on improving patient outcomes by engaging patients, coordinating care and referrals, addressing social determinants of health, and advocating for patient needs.   KEY RESPONSIBILITIES: The following duties and responsibilities generally reflect the expectations of this position but are not intended to be all inclusive.  Other duties may be assigned. • Care Coordination/ Engagement:  - Conduct active engagement campaigns to educate and drive patient utilization of available programs and clinical resources.  - Develop and implement individualized care plans in collaboration with the patient, their family, and other members of the healthcare team (Providers, Nurse Practitioners, Physician Assistants, physicians, nurses, therapists, and other professionals).  - Schedule and coordinate appointments, referrals, and other necessary services.   - Monitor patient progress and identify potential barriers to care.   - Facilitate communication and information sharing among members of the care team.  - Ensure timely receipt and processing of authorizations and referrals. • Patient Advocacy:  - Advocate for patient needs/rights, facilitating access to care and appropriate resources.  - Understand employer benefits, and/or employee incentive plans, available to patients (employees and dependents).  - Identify and address social determinants of health that may impact patient outcomes (e.g., housing, transportation, food insecurity).  - Assist patients with navigating the healthcare system,   - Empower patients to make informed decisions about their care. • Communication and Documentation:  - Maintain accurate and up-to-date patient records in the electronic health record (EHR) as appropriate.  - Communicate effectively with patients, minor patients’ guardians, and other healthcare providers, both verbally and in writing.  - Prepare reports and presentations as necessary for administrative and client needs. • Quality Improvement:  - Participate in quality improvement initiatives to enhance patient care and operational efficiency.  - Identify areas for improvement in care coordination and patient experience.   WHAT'S REQUIRED? • Current Tennessee licensure as a Licensed Clinical Social Worker (LCSW), Licensed Practical Nurse (LPN), or Registered Nurse (RN) • ASN, BSN, or MSW, depending on licensure type • 5+ years of experience in healthcare, case management, patient advocacy or a related field.   THE IDEAL CANDIDATE WILL HAVE: • Strong communication, interpersonal, and organizational skills. • Ability to build rapport and establish trusting relationships with patients. • Excellent problem-solving and critical thinking skills. • Proficiency in using electronic health records (EHR) and other relevant software. • Knowledge of medical terminology and healthcare regulations. • Ability to work independently and as part of a team. • Strong customer service orientation. • Ability to handle sensitive information with discretion and maintain confidentiality. • Motivational Interviewing and Appreciative Inquiry   BENEFITS INCLUDE: • Competitive Salary* • Medical, Dental, and Vision Options • Retirement savings plans • Paid Time Off • and MORE!   *The hourly rate range for this position is $36.06 to $45.67 per hour.  It is not typical for an individual to be hired at the top of the range, as compensation is determined on an individualized basis and will be impacted by location, experience, expertise, internal pay equity, and other relevant business considerations.   ABOUT URGENT TEAM: The Urgent Team Family of Urgent Care & Walk-in Centers, an “on-demand” healthcare company, was created to provide quality, affordable and, convenient walk-in medical and telemedicine service in suburban neighborhoods, rural towns, and mid-size cities across the Southeast.   All of our 80+ centers are have achieved the Urgent Care Association Accreditation, the highest level of distinction for an urgent care center.
