Care Coordination Support Specialist

Summary of Primary Function

This position will provide direct administrative assistance to the Care Coordination Team that promotes patient care and services.  Will interact with patients at times at the direction of a licensed Care Coordination team member to provide assistance in scheduling and accessing information. 

               

Essential Functions

Provides community based support and assistance to patients and staff to implement care plan as deemed necessary by the Care Coordination Team.Assist clinical and administrative staff in for preparing patient appointments (pre-visit planning)Act as a resource to patients and families, providing answers to frequently asked questions and suggesting educational materials using Care Coordination Team ProtocolsMaintain comprehensive progress notes for each patient interactionMaintains good working relationships with and knowledge of applicable community servicesUtilize interpersonal skills and community resources to effectively advocate for individual in the communities servedMaintains required data and documentation and prepares required report for the Care Coordination TeamPerform duties with minimal supervision.  May be required to occasionally work irregular hours, attend other job related meetings, and perform other duties as assignedThis document is not an exhaustive list of all responsibilities, skills, duties, requirements, or working conditions associated with the job. Employees may be required to perform other job related duties as required by their supervisor, subject to reasonable accommodation.Patient PopulationDemonstrates the knowledge and skills necessary to provide care appropriate to the age of the patients served on his or her assigned unit.  Demonstrates knowledge of the principles of growth and development of the life span and possesses the ability to assess data reflective of the patient's status and interprets the appropriate information needed to identify each patient's requirements relative to his or her age, specific needs and to provide the care needed as described in departmental policies and procedures.

Education Qualifications

Required Minimum Education: HS or GED

    

Preferred Education: Associates Degree

Specialty/Major: Healthcare or LPN

Licensing/ Certification

Licensure/Certification Required: None

Licensure/Certification Preferred: Medical Assistant or higher

Minimum Qualifications

Minimum Years and Type of Experience

1 year experience in community or ambulatory care based setting.

Other Knowledge, Skills and Abilities Required

Works well in a Team Setting. Excellent interpersonal communication skills. Strong analytical, data management and computer skills.  Experience with database entry. Highly organized and detail oriented. Accepts responsibility and follows through on projects and activities

Experience in a healthcare related environment working with patients, families, and providers
