As a Care Coordinator, you will:

Assist clinical case managers by working with clients, gathering, compiling and coordinating continuing care informationProvide AA/NA contacts in clients home area, set up interviews, arrange transportation and contact agencies with regard to clients continuing care planMaintain the continuing care database, with special emphasis on continuous updates of clinical value and workWork with the admissions office and provide back up for admitting clients, when needed, as well as provide the continuing care chart checks for all patients leaving treatmentFollow up phone calls within 5 days of discharge for WI client facility placements for continuing care services. Qualifications

Required  

High School Diploma or equivalent1 or more years in customer service; intermediate level experience with Word software, including Outlook, Word, PowerPoint; in online research

Preferred 

Vocational or associates degreeExperience with patient databaseBasic knowledge of chemical dependency, including 12 step programs and the MN modelExperience in a behavioral care setting. Overview
