Primary Job Functions:

General Responsibilities:

Liaison between HIV Prevention and Outreach/Community Agencies and ProgramsProvide community outreach and HIV/HEPC prevention educationProvide HIV and HEPC testing clinics identified at risk populations within targeted communitiesAdminister oral swab test kits within community outreach clinics, and properly dispose of medical waste according Accordia Infectious Control PolicyProvides ongoing monitoring and care coordination for patient’s referred to chronic care and PrEP servicesAssists with meeting reporting requirements of the HRSA and UAB grantsProvides support and advocates on behalf of patients, to secure community resources and needed servicesEffectively collaborates and consults with staff members, including clinicians and pharmacists, and outside sources in the delivery and arrangement of HIV/HEPC Prevention Services to targeted populationsProvides education on virus management and healthy living skills within educational outreach groupsDemonstrates the ability to recognize the elements of a crisis state and knows how to deescalate or resolve the situationProvides appropriate linkage to chronic care (Ryan White Centers) and PrEP services within communityEffectively demonstrates the application of HIV/HEPC Prevention and Outreach specific competencies

Bachelor’s degree in a behavioral health, social services, or public health related discipline preferred. A certification of case management training by Alabama State Department of Mental Health and experience within the Care Management field preferred or work related experience; valid verifiable state-issued driver’s license; knowledge in high-risk care specialties, outlined below, to be attained within six (6) months of employment:

 

Knowledge of chronic medical conditionsBasic knowledge of growth and development of child consumers as related to all intellectual, emotional, spiritual, and physical aspects consumer developmentKnowledge of cultural diversityKnowledge of Trauma Informed Care principlesKnowledge of behavior management, crisis intervention and interventions common to acute psychotic consumers, as well as, to non-violent crisis intervention practices.Knowledge of HIV education and prevention methodsKnowledge of HIV and HEP C viruses and PrEP/ PEPKnowledge of social determinants of healthKnowledge of available referral sources for physical health, mental health, and social servicesBasic knowledge of utilizing Electronic Health RecordsBasic knowledge of DMH and Medicaid standards
