Job Details:
The Care Coordinator is responsible for the managing the case coordination for patients assigned to them. This includes ensuring an excellent level of care is being provided to their patient base regarding their infusion pharmacy services.
Responsibilities
Maintain complete, organized and accurate files for all patients relating to coordination of services: case management, nursing services, pharmacy preparation and delivery, and any data significant to the overall patient case coordination.
Responsible for the initial input of data on all patients and prior documentation relating to case coordination. Informational data should be from the point of patient notification through verification of benefits, initiation of care, first day of therapy follow-up, completion of services, discharge and cessation of communications.
Timely completion of all daily, weekly, monthly, quarterly and annual reporting requested or required by the Intake Manager, General Manager or ProHealth.
Strict adherence to all policies & procedures ensuring that case coordination of patient therapy and staff involved in their respective processes are complying with state guidelines and maintaining current working knowledge of all areas.
Identification and resolution of any and all case coordination complaints, concerns or deficiencies with written documentation being given to the Intake Manager.
Professional and courteous interface with internal and external personnel participating in patient case coordination. This would include, but not be limited to: hospital discharge planners, home health agency staff, physicians, physician office personnel, case managers, health benefit providers, hospital employees, center team members and all other referral sources.
Participation in general and departmental specific orientation of new center staff to include overall referral process, computer and paperwork documentation, cross-training and work duties.
Verification and training, when applicable and directed by departmental manager, of home health, case management, hospital, extended care, physician office, clinic, and other health professionals to assure current working knowledge for performance and understanding of home infusion care. This includes, but is not limited to, assisting with educational assessments, presentations, sales calls and professional organization meetings.
Active involvement in the QA&I activities: monitoring assigned outcome indicators and working toward achievement of established goals and expectations for the Beyond Expectations program.
Maintain the confidentiality of patients and their personal information at all times.
Participate in any continuing education and in services as requested by management
Other duties as assigned.
Qualifications
High School diploma or GED
One year of healthcare, contact center or medical experience in the home health or home infusion field
Two years of home infusion field with strong clinical knowledge will be considered
Proficiency in the Microsoft Suite
