DESCRIPTION OF POSITION The Accountable Care Organization (ACO) nurse coordinates team based care to provide, develop, implement, and manage programs set by Centers for Medicare & Medicaid Services (CMS) and our ACO consortium. These services deliver evidence-based practice that are provided to individuals, families, and/or their communities through effective partnerships with patients, their caregivers, and their physician to achieve high quality care that is patient/family centered. QUALIFICATIONS

 

Professional/Educational BSN required, MSN or APN preferred. 3-5 years' experience in a clinical or community health setting. Previous Care Coordination and/or Case Management experience preferred. Previous experience with health IT systems and data reports preferred. Previous experience with mobilizing community resources, navigating through the healthcare continuum and working with disparate populations preferred. Ability to speak a relevant second language preferred. DCFS Mandated Reporter Certificate of Completion required.

 

Personal Demonstrates evidence of essential leadership, communication, education and counseling skills. Proficient in communication technologies (e-mail, cell phone, etc). Effective organizational skills, demonstrates ability to maintain accurate notes and records. Core Values consistent with a patient/family centered approach to care. Demonstrates professional, appropriate, effective written, verbal and nonverbal communication skills.  Demonstrates a positive attitude and respectful, professional customer service.

 

Actual pay will be determined by qualifications, experience, and internal equity.
