Minimum Qualifications
The Care Coordinator plays a vital role in supporting students who experience various forms of personal or academic challenges, including problems with access to basic needs such as food, housing, healthcare, or other resources, as well as other situations causing distress or barriers to success. Serving as a member of an interdisciplinary support team, the Care Coordinator will manage a caseload of referred students and work to provide comprehensive support to referred students by assessing their situation and coordinating connection to resources both on and off campus. This position reports to the Assistant Dean of Students.
Primary Responsibilities:
Assessment and Support
· Provides consultation, case management services, and coordinates care for students of concern referred to the CARE Team by the campus community.
· Conducts thorough assessments, including social histories, motivational interviewing, and relevant instruments (NABITA Risk Rubric, NAS, AUDIT, PHQ-9, etc.) to understand the individual's needs and experiences. Tracks student progress and maintains records.
· Collaborates with the Care Team to create personalized care plans based on the assessment, ensuring that the individual's needs are addressed.
· Organizes and coordinates various services such as counseling, medical assistance, legal support, and academic accommodations.
· Liaises with the campus and community partners to ensure clear communication about appropriate resources to students.
· Maintains regular contact with the student to monitor progress and adjusts the care plan as needed; maintains accurate records of student interactions, interventions, and progress within the case management system.
Advocacy
· Acts as an advocate for the student, liaising with relevant campus and external resources to ensure the student's rights and needs are met.
· Collaborates with other Student Affairs staff and members of the campus community to ensure a holistic approach to supporting the individual and provides guidance on navigating campus resources and policies.
· Connects students with relevant campus services like tutoring, counseling, financial aid, disability services, as well as community organizations based on their needs.
· Monitoring referrals through Single Stop database, which includes a wide range of services, such as government benefits, healthcare, legal assistance, financial counseling, housing support, food assistance, and more.
· Develops relationships with external organizations or agencies to collaborate with our campus community to provide additional support and resources.
· Responds to immediate student crises, providing support and coordinating emergency response when needed.
Education and Awareness
· Creates promotional materials for basic needs events, activities and services.
· Conducts educational programs and workshops to raise awareness about relevant issues and available support services.
· Promotes community health by educating individuals on preventive measures and healthy behaviors.
· Advocates for campus policies that support wellness.
Qualifications, Knowledge, Skills, and Abilities Education and Experience:
· Required: Bachelor’s degree in Social Work or related field from an accredited institution or two years of professional experience in higher education or case management.
· Preferred: Master’s degree in Social Work, Counseling, Higher Education, or a related field from an accredited institution.
· Preferred: Certified Case Manager (CCM) or relevant professional license
Required Knowledge, Skills, and Abilities:
· Demonstrated sensitivity to, and understanding of, the diverse academic, social, economic, cultural, disability, and ethnic backgrounds of college students.
· Knowledge of community and state resources, how to make appropriate referrals to community and other agencies for services, and ability to coordinate services.
· Strong assessment, critical thinking, and crisis intervention skills.
· Strong communication skills, as well as the ability to work independently while developing strong collaborative relationships with colleagues, students, student/academic services, faculty, and community partners.
· Strong organizational abilities to manage multiple tasks and deadlines.
· Familiarity with student information systems and data analysis
· Preferred: Experience with Maxient database
