**Responsibilities**
The registered nurse (RN) is accountable for the coordination of nursing care, including direct patient care, patient/family education and transitions of care. The RN supports professional nursing practice across practice settings and across the continuum of care to meet the needs of the patient and family. The RN will function within the Mayo Clinic Nursing Professional Practice Model, which includes accountability for planning, implementing, evaluating and communicating all phases of nursing care for assigned patients. The ANA Nursing: Scope and Standards of Practice provide a basis for the practice of the RN. The RN provides leadership through activities such as preceptor role, informal and formal leadership roles, and quality improvement efforts. The RN delegates patient care according to skill level, experience, patient acuity, fiscal accountability and availability of resources. The RN possesses excellent communication skills; is skillful in mentoring and teaching; and may participate on committees or projects. A subset of employees may be required to drive their personal vehicle as a part of the responsibility of their role.

**Qualifications**
This position is a Care Coordination role within the Family Medicine practice for the Arrowhead and Thunderbird Ambulatory Clinics. The primary responsibility is care coordination for a specific, complex patient population seen by the Family Medicine practice that requires Nurse Coordinator oversight. This role involves managing the healthcare needs of this same group of patients, including scheduling, order review, and collaborating with all aspects of the multidisciplinary team.
This position offers a significant amount of autonomy, while still working within a collaborative team environment. The nurse will be solely responsible for managing all aspects of care for this group of patients, ensuring that their needs are met through coordination and direct involvement. This role will require time spent at both the Arrowhead and Thunderbird Ambulatory Clinics, with scheduling and responsibilities split between the two sites based on operational needs.
**Education:**
+ Graduate of a baccalaureate nursing program accredited by one of the following:
+ Accreditation Commission for Education in Nursing (ACEN)
+ Commission on Collegiate Nursing Education (CCNE)
+ National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA)
+ If the nursing program was not accredited by ACEN, CCNE, or NLN CNEA, at least one year of RN experience in an applicable care setting or one year of LPN experience at Mayo Clinic is required.
+ All entry-level associate degree registered nurses with an RN start date of April 1, 2020, or later must provide documented evidence of completing the baccalaureate degree in nursing from an accredited program within five years from their RN start date.
**Experience:**
+ **Preferred:** Primary Care experience
+ At least **2 years** of nursing experience required
**Skills:**
+ Excellent **communication skills** (both verbal and written).
+ Ability to work effectively in a **team environment** .
+ **Computer skills** required; prior experience with **electronic medical record (EMR) systems** preferred.
+ Ability to work **flexible hours** , including days, evenings, nights, weekends, holidays, and on-call as needed.
+ Ability to **adapt to unpredictable situations** within the work setting.
+ Demonstrated **leadership skills** , with an ability to communicate effectively and apply **critical thinking** in decision-making.
**License and Certifications:**
+ **Licensure:**
+ Current **RN license** as required by applicable state regulations.
+ **Certifications:**
+ **Basic Life Support (BLS)** certification for Health Care Providers from one of the following:
+ American Heart Association
+ American Red Cross
+ BLS certification must be valid for 90 days from the start date or renewed before the start date.
+ **Additional state licensure** and/or **specialty certifications** may be required, depending on the specific work area.
