Care Coordinator

GENERAL  DESCRIPTION

 

The Care Coordinator reports to the Integrated Health Home (IHH) Program Manager, and provides and arranges for the provision of services necessary to enable clients to achieve optimal whole health.  This position assists clients in identifying, coordinating, securing and sustaining resources necessary to live a healthy lifestyle and have positive whole health, including wellness, behavioral and physical health.

CHARACTERISTIC  DUTIES

Conduct community resource assessments and referrals; refer to community based services as needed and

complete necessary linkage documentation.

Coordinate and manage enrollment processes for clients into the IHH services.

Assess current physical health care, behavioral health care and substance abuse support as applies.

Collect and assess outcomes from the coordination of the JHH services.

Identify, communicate and work collaboratively with behavioral health and medical providers, and other community support providers to facilitate care and integrate resources to assure whole health needs are met.

Evaluate clients for informal and formal support systems on an on-going basis.

Provide individualized care planning and mutual goal setting with clients.

Assist in the development and updating of crisis plans and incorporate into individual care plan.

Provides crisis support as needed.

I 0.   Provide  continued  education  and  reinforcement   to  clients  and  their  families/care  advocates   in  whole  health.

Participate in care coordination activities, case discussions and quality improvement efforts.

Monitor and communicate service outcomes.

Schedule, arrange for and/or provide transportation to appointments as needed.

Complete follow-up communication with clients after appointments.

Complete required documentation for assigned caseload.

Provide support to other IHH staff as needed.

Other duties as assigned.

PERFORMANCE STANDARDS

Consistently demonstrates ability to meet requirements specified in Integrated Health Home program.

Works in productive and efficient manner to promote reaching client whole health goals.

Engages behavioral health and physical health providers in collaborative client whole health management.

Ensures client records and electronic data are current and maintained appropriately.

Provides excellent customer service through positive verbal and written communications.

Proactive in assisting and/or directing IHH clients with whole health services.

IHH assigned client caseload is maintained and their whole health has increased positive outcomes.

Stays abreast of all IHH governmental changes and communicates such to supervisor(s).

MINIMUM QUALIFICATIONS

I.    Bachelor's Degree in Human Services field.

 Experience working on a multi-disciplinary team.

Ability to effectively and professionally manage difficult client issues

Strong organizational, written and verbal communication skills

Valid driver’s license, regular availability of personal car, and evidence of insurance coverage. 

Good driving record as determined by BMC requirements

Ability to meet clients in their homes or in public places with ability to drive clients safely, and a willingness to transport clients in own vehicle as needed.

Experience working with Microsoft Office software. PREFERRED  OUALIFICATIONS

Successful targeted case management or community based services experience
