Shift Monday – Friday, Day Shift; primarily 8:00 am – 5:00 pm (40 hrs/wk); other hours/schedule as needed Job Summary This position is located off-campus in Lexington, KY at UKHC Bluegrass Care Clinic.

The Care Coordination Manager will supervise all clinical and non-clinical staff functions in the Bluegrass Care Clinic. Ensuring appropriate management of resources and quality patient care are essential job duties for this position.

This includes initiating actions for resolutions, maintaining nursing standards of care and magnet-related designation requirements, and participating in updating policies and procedures. In addition, this position will be responsible for ensuring adequate nursing staff and clinical support staff are available to care for patients, working with staff to develop schedules and coverage plans, acting as a clinical and systems resource for staff, and completing designated staff performance evaluations.

This position will work directly with the ambulatory leadership team for practice, operations, clinical improvements, and nursing initiatives.
