Care Coordinator - BMC Mobile Crisis - Full-time Days
Care Coordinator reports to the department area manager and provides discharge planning services, psychosocial evaluation, counseling and support to clients admitted to the Crisis program area. The individual works in collaboration with other department area staff to provide a full array of Behavioral Health support.
Schedule
This position is generally 7:00 am - 5:30 pm.
Qualifications
Bachelor’s degree in human services field from an accredited educational program2+ years working with individuals with chronic mental illnessAbility to work cooperatively with the multi-disciplinary teamExcellent written and verbal communication skillsAbility to verbally and physically de-escalate a client utilizing BMC trained programsWork Shift
