Care Coordinator Population Health
Job Description​

 

We are hiring an RN Care Coordinator to support our Primary Care Clinics. This is a full-time day shift position.

The RN Care Coordinator is responsible for the application of extensive knowledge in clinical performance improvement activities

This individual will be responsible for driving engagement strategies for improving physician and staff patient care practices based on clinical guidelines, best practices internal and external benchmarks, and claims based data.The Care Coordinator supports and ensures the overall success of the patients healthcare by collaboratively working with patients physicians practice teams and health plans to select guide and direct patient care while integrating key features of the medical home

Important to Note:

Dual License RN required - KS and MO RNBLS certification requiredPosition Hours: Monday - Friday, 8am - 5pm.Position will work in and support Physician Clinics

Job Requirements

Applicable Experience:

3-5 years

Registered Nurse (KS) - National Council of State Boards of Nursing (NURSYS), Registered Nurse (MO) - National Council of State Boards of Nursing (NURSYS)

Bachelor's Degree
