The Social Worker I will provide care coordination services to patients and their families)referred from the interdisciplinary teams in the outpatient medical clinics. The incumbent will provide assessment of needs, development of plans, support and assistance in linking to resources and overcoming barriers to access, advocacy, and psychoeducation. This position may engage in other social work roles as assigned and within the scope of practice for a Licensed Bachelor Social Worker (LBSW).
Responsibilities Accept referrals for care coordination services according to the process designated by the Manager, Clinical Social Work in the outpatient medical clinics.Utilize protocols to gather information, assess needs, maintain a record of contacts, and provide a record of case activities and status through referral database.Complete and submit documentation for the medical record to supervisor for approval in a timely manner.Adhere to standards of care and continuing education outlined in the Case Management chapter 10.42.02 Code of Maryland Regulations.Establish and maintain contact with the referring clinician, the patient, and the patient’s family while cases are open.Collaborate with administrative staff to update and maintain resource and referral databases.Attend and participate in staff meetings and trainings.Participate on committees and workgroups within the Center for Developmental Behavioral Health (DBH) and Social Work departments as assigned.Participate in implementation of interventions (e.g., group sessions) with approval of supervisor and in collaboration with the facilitating clinician. Qualifications
Must possess and maintain Licensed Bachelor Social Worker (LBSW) licensure issued by the Maryland State Department of Health and Mental Hygiene Board of Social Work Examiners.
EDUCATION:
Bachelor’s degree in Social Work from a program accredited by the Council on Social Work Education required.
EXPERIENCE:
One year of experience providing care coordination services preferred. This may include licensed, non-licensed, and/or internship experience.Experience in a medical or mental health setting preferred.
