CARE Director

Position Summary: The CARE Director is responsible for overseeing consumer access operations, referral network development, and intake coordination across behavioral health, substance use disorder (SUD), and integrated care services. This role includes supervision of access/intake staff, oversight of care coordination and navigation activities, and collaboration with both internal teams and external stakeholders to improve timely entry into services.

 

The CARE Director plays a key role in supporting the agency’s transition toward an integrated care model that includes mental health, SUD, physical health coordination, and social support services. The position also includes limited direct service responsibilities under supervision for individuals pursuing licensure.

 

Essential Duties and Responsibilities

· Oversee access operations including triage, intake coordination, and service linkage across behavioral health, SUD, and integrated care programs

· Supervise access and intake staff; ensure compliance with agency protocols, regulatory standards, and documentation requirements

· Coordinate referrals and establish ongoing relationships with hospitals, primary care providers, courts, schools, MCOs, and other community-based partners

· Address systemic access barriers and recommend workflow improvements to support integrated service delivery and health equity

· Oversee care coordination and navigation functions to ensure continuity of care across behavioral health, primary care, and social service domains

· Provide limited direct services under clinical supervision (within CADC/LCADC intern scope), including screening, engagement, and psychoeducation

· Participate in interdisciplinary meetings, case reviews, and clinical supervision

· Track access and referral data, prepare reports for quality improvement and contractual compliance

· Represent the agency at external stakeholder meetings, provider coalitions, and outreach events

· Support the implementation of screening and navigation tools for social determinants of health (e.g., PRAPARE, I-HELP)

· Perform other duties as assigned
