Care Guide - 2 Positions Available!
The Care Guide provides support for a targeted group of patients to help them navigate their treatment within the organization. This role serves as a patient's advocate, working with the primary care team to improve quality of care and health outcomes for patients, as well as ensure providers and staff are operating at the top of their license and capabilities.

Provides scheduling assistance to patients, reaching out to patients who have upcoming appointments to ensure they arrive at their appointment on time enabling office-wide efficiency.
Performs tasks such as arranging transportation for patients who would otherwise not be able to attend appointments to support patient and community health.
Serves as a champion for patients, ensuring that primary care team is providing patients with the optimal care.
Partners with the primary care team to enable patients to be connected to both community and network-based health support systems.
Communicates effectively and kindly with patients, families and providers alike to increase understanding and ensure external parties have a positive experience with the organization.
Performs other duties as assigned.
EDUCATION:Required: High School Diploma or equivalent
EXPERIENCE:Required: 2 years of experience in Care Management or equivalent university studies course work
