Under the direction of the Case Management leadership team, will work collaboratively with the Social Workers (SW), RN Care Managers (RN-CM), Utilization Review RN's (UR-RN) and Payer Denial Specialists by performing multiple administrative supportive duties that facilitate a seamless transition of patients from the inpatient hospital setting, using current knowledge of hospital processes and community services. This position requires frequent contact with patients and families including but not limited to hospital staff, community resource agencies and payer representatives.

Required Qualifications:
Bachelor's Degree. 2 years of experience in customer service, patient education, and/or patient engagement.Preferred Qualifications:
