Schedule: Full Time

Shift : Monday - Friday - 8A - 430P

The role you’ll contribute:

The Care Management Assistant (CMA) is responsible for care navigation activities designed to ensure a smooth transition of care for our patients within AdventHealth Hendersonville as directed by the care team.

The value you’ll bring to the team:

When directed by an RN Case Manager or Social Worker assists with the following discharge planning tasks

Assists with home care referrals ensuring forms are completed correctly and within regulatory compliance.

Checking for nursing home, rehab, adult foster care and other facility vacancies/placement

Assists setting up transportation

Makes referrals for community agencies

Assists Care Obtains discharge appts as directed by the physician and the care team.

Provides outpatient providers with necessary medical records.

Communicates with patient/families, payers, HH staff and outside providers

Optimizes transitions of care through the following:

Schedules outpatient follow-up appointments, procedures and therapies as directed by the physician and/or the care team.

Documents the patients/family’s agreement and understanding of schedule

Answer questions related to their discharge and/or forward’s questions to appropriate discipline.

The expertise and experiences you’ll need to succeed:

Minimum qualifications :

EDUCATION AND EXPERIENCE REQUIRED :

High School diploma or equivalency

Minimum of one year office or customer service experience, preferably in a medical setting

KNOWLEDGE AND SKILLS PREFERRED :

Knowledge of medical terminology

Certified Medical Assistant Experience preferred

Knowledge of Microsoft Office (Word, Excel and Power Point)
