Care Management Behavioral Health Professional

 Humana Healthy Horizons in Florida is seeking a Care Management Behavioral Health Professional who will help patients dealing with issues such as addiction, abuse or mental illness by providing behavioral health services. The Care Management Behavioral Health Professional’s work assignments are often straightforward and of moderate complexity.

The Care Management Behavioral Health Professional will work with members in the case management program, engaged in ABA services (Applied Behavioral Analysis). The Care Management Behavioral Health Professional will outreach to members/guardians to engage them in the Care Management Program and will screen member for social determinants of health (SDOH), medical and other identified needs, closing gaps in care. The Care Management Professional will provide members with community resources, attend case conferences with treatment providers to address clinical progress as needed, and will work with member/family to ensure all gaps are closed related to ABA and member’s needs. The Care Management Professional will screen and assist with other needs such as but not limited to: Speech/OT/PT, Psychological, Neuropsychological testing.

Employs a variety of strategies, approaches and techniques to manage a member's physical, environmental and psycho-social health issues.Identifies and resolves barriers that hinder effective care. Ensures patient is progressing towards desired outcomes by continuously monitoring patient care through assessments and/or evaluations.May create member care plans.Understands department, segment, and organizational strategy and operating objectives, including their linkages to related areas.Makes decisions regarding own work methods, occasionally in ambiguous situations, and requires minimal direction and receives guidance where needed.Follows established guidelines/procedures.

Required Qualifications:

Bachelor’s degree in nursing, social work, or behavioral health related field OR 3+ years of experience working with individuals receiving ABA (Applied Behavioral Analysis) services.Registered Behavior Technician with 2+ years’ experience.AHCA Level II Background Check is required.

Preferred Qualifications:

Experience working in managed care.BCBA (Board Certified Behavioral Analyst).Care Management/Care Coordination experience.RN or LPN with experience working with children with autism spectrum disorder.Master’s- LCSW, LMHC with experience working with children with autism spectrum disorder.

Additional Information

Workstyle: Home. Home workstyle is defined as remote but will use Humana office space on an as needed basis for collaboration and other face-to-face needs.  Work Location: preferred to reside in FloridaTypical Work Days/Hours: Monday – Friday, 8:30 am – 5:00 pm EST.

Pay Range

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.
