About the Role:

Roles and Responsibilities:

Develop, organize, and manage care facilitation services (e.g., Case Management, Social Work, Utilization Management, post-acute care coordination)

Lead Hospital Case Management programs to improve patient progression and physician collaboration

Shape patient care experiences through effective care-plan interventions

Job Requirements:

Current state RN License

5 years of case management experience in a healthcare setting

Strong communication and customer relations skills

Preferred Qualifications:

Master’s degree

3-5 years of supervisory experience

Formal case management training
