Sheltering Arms Institute is seeking a Care Management Specialist PRN to join our team and provide both weekday and weekend coverage. Sheltering Arms Institute’s Care Management Team provides both direct patient care to patients and families and assist in the coordination of team delivered services. The Care Management Specialists complete a psychosocial screening, followed by a more thorough assessment as needed to contribute to the patient’s individualized transdisciplinary treatment plans, supporting the patient and family’s goals. These providers offer psychosocial support and intervention; educate patients, their family members, team members, and social work students; facilitate compliance with treatment; work with the community to provide needed services; and coordinate discharge planning activities. The Care Management Team plays a vital role in coordinating the movement of all patients through their inpatient stay at Sheltering Arms Institute, while honoring the treatment goals and plan of each individual patient. This effective and attentive planning for life beyond inpatient rehabilitation is also critical for SAI to meet organizational goals associated with our Mission, Vision, and Values. Care Management specialists work directly with patients, families, the rehab team, third party payers, and external agencies to assure the development of a coordinated rehabilitation program that will maximize patient outcomes and create a safe, sustainable discharge plan. Care Management serves as a resource to other team members regarding prudent utilization practices and payer requirements.
Qualifications and Skills
Previous experience in a healthcare setting providing clinical social work and/or case management services required. Acute care or inpatient rehab experience preferred. Experience in an academic healthcare setting and/or clinical experience with specialty patient populations (Traumatic Brain Injury, Spinal Cord Injury, and Stroke) also preferred.Education/Certifications
Master's Degree of Social Work from a Council on Social Work Education accredited program or Baccalaureate Degree in Nursing from an accredited School of Nursing preferred Registered Nurse in Virginia or License required if a licensed registered nurse Licensed Clinical Social Worker preferred Certified Case Manager certification preferredQualified applicants will receive consideration for employment without regard to race, color, religion, sex, national origin, sexual orientation, gender identity, disability or protected veteran status.
