Position Purpose:
The Care Management Support Coordinator II provides vital administrative support for care management activities. This role involves conducting outreach, managing inbound calls, scheduling services, and acting as a liaison for members, providers, and internal staff. The coordinator ensures compliance with state and regulatory guidelines while maintaining high-quality member and provider interactions.

Key Responsibilities:

Conduct outreach to members by phone to assist with care plan next steps, address questions, and provide ongoing education about health plan and community resources. Resolve inquiries and concerns from members and providers, including explaining care plan procedures and protocols. Facilitate member onboarding by sending welcome letters, program materials, and correspondence to foster strong member/provider relationships. Document and maintain non-clinical member records in compliance with state and regulatory standards. Connect members to health plan and community resources to address Social Determinants of Health (SDOH) and enhance care quality. Support daily administrative tasks, including scheduling, member communication, and reporting. Ensure compliance with organizational policies and standards while performing all duties. Perform additional duties as assigned.

Education and Experience Requirements:

Required: High School Diploma or GED 1-2 years of relevant experience Customer service skills, including call handling and issue resolution Preferred: Some college education Healthcare industry experience Familiarity with performance metrics

Technical Skills:

Proficiency with MS Office Suite (Word, Excel, Outlook, etc.) Ability to navigate and use member care systems after training

Performance Expectations:

Complete a minimum of 25 member outreach calls daily. Maintain accurate documentation of member interactions and records. Deliver high-quality service in alignment with care management goals and regulatory standards.
