Care Management is a collaborative practice model including the patients, nurses, social workers, physicians, other practitioners, caregivers and the community. The care management process encompasses excellent communication, both verbal and written, and facilitates care along a continuum through effective resource coordination and addressing the psych social needs of the patient.

Required qualifications:

Master's Degree - Social Work, Psychology, or Counseling or equivalent education/experience or Master’s degree with 5 years relevant social work experience Upon request: Driving may be necessary as part of this role. Caregivers are required to comply with all state laws and requirements for driving. Caregivers will be expected to provide proof of driver license and auto insurance upon request. See policy for additional information. 6 months of Clinical experience in a HMO, medical group, affiliated model, hospital or medical office/clinic setting.

Preferred qualifications:

3 years of Experience in utilization management or case management Experience with psychiatric chemical dependency treatment Managed care experience
