Care Manager RN

The Care Manager RN is responsible for managing patient populations with chronic illness and/or multiple comorbidities.  This individual works in coordination with multiple PCPs and serves as the central link to an identified population of patients.

Required qualifications:

Graduation from an accredited school of nursing.Upon hire: Washington Registered Nurse License 2 years of work experience in RN role.

Preferred qualifications:

Bachelor's Degree in Nursing.Experience managing complex patients in an ambulatory setting.Medical Home Model experience.
