Summary:
The Care Manager R.N. reviews the admission and continued stays of all patients in the Case Management Program, the purpose of which is to assure high quality patient care and effective utilization of available health facilities and services. He/she is responsible for assessing the appropriateness of the level of care, diagnostic testing and clinical procedure performed; quality and risk issues; and documentation within the medical record. Information is abstracted from the patient chart and forwarded to Quality Improvement, Infection Control and other departments as necessary. He/she refers cases not meeting screening criteria to the appropriate physician advisor.
Experience/Education Requirements:
Graduate of an approved school of nursing, BSN preferred. A minimum of 5 years medical/surgical experienceExperience/certification in critical care nursing is preferredPrevious experience with Case Management and Utilization Management is preferredLicense or Certification Requirements:
