The Patient Care Navigator will assist patients with chronic conditions to adopt healthy behaviors through home and/or phone visitation. Patient Navigators work collaboratively with physicians, staff and other health care professionals to provide evaluation and assessment of patient needs for care coordination across the health care continuum.

Required Qualifications:

Graduate of an accredited licensed vocational school of nursing. Or Bachelor's Degree Social Work; from an accredited school of social work. upon hire: Texas Licensed Vocational Nurse. 1 year Experience with customer service, patient education and or patient engagement.

Preferred Qualifications:

1 year Experience in a health care facility.
