Care Specialist
Aleph Older Adults is looking for a Care Specialist (hereafter referred to as “Mentor”) that will provide guidance and emotional support to a client with cognitive impairments and disabilities. The Care Specialist will assist the client in developing the physical, intellectual, emotional, and social skills necessary for independent living. This is full-time, benefts eibile, hibrada role. The Care Specialist’s responsibilities include providing consistent support throughout the week and being available and on-call during all evenings, weekends and holidays. + Provide support to the client through phone calls and in-person visits as needed. + Ensure the client’s daily schedule and requirements are met. + Build a professional, supportive relationship with the client, serving as a role model and resource. + Help the client establish connections within the wider community. + Resolve disagreements and conflicts involving the client and community members. + Recommend and facilitate participation in local events/activities. Print, review, and plan the client’s weekly calendar, including required appointments or activities. + Encourage and celebrate the client’s successes while holding them accountable in a supportive manner. + Offer resources, strategies, and constructive feedback to help the client achieve their goals. + Support the client in learning job skills, demonstrating appropriate work behaviors, and navigating workplace culture when necessary. + Communicate daily with the client through phone calls or in-person meetings. This includes after hours, weekends and holidays. + Provide regular updates to the client’s family, addressing any questions about their care and well-being. + Coordinate with healthcare professionals to schedule and follow up on appointments. + Accompany the client to medical appointments and hospital visits when required. Review care plans after doctor’s visits and update as needed. + Maintain accurate documentation of all interactions and communications with the client, ensuring compliance with organizational standards. Protect the client’s privacy by securely managing records of their goals, progress, and notes. Required Education & Experience: + BS in the field of human or social services, social work or other appropriate degree. + Two+ years' experience working in a residential facility or in home healthcare is desired. + Experience in principles-based youth work and leadership. + Ability to build relationships with different types of people, including clients, organization members, and members of the health care team. + A valid driver's license and a satisfactory MVR.
