CARE Support Coordinator

Position Summary:
The CARE (Campus Assessment, Response, and Evaluation) Support Coordinator is a part of the CARE Support and Intervention Team within Student Care Coordination in Student Affairs at Vanderbilt University and is a key individual contributor responsible for supporting coordinated and holistic support measures for students of concern, specifically those with needs that are high in acuity and those taking a medical leave of absence (MLOA). Reporting to the Associate Director of CARE Support andIntervention, the coordinator will routinely coordinate with entities of the CARE Support and Intervention team, Student Care Network, Student Affairs staff, academic units, and other relevant departments to address the needs of students and maintain regular contact with students and their parents, as needed. 

The CARE Support Coordinator will help facilitate outreach to students who may have high acuity needs, assist with accessing on and off-campus resources, refer to the appropriate level of care needed. The CARE Support Coordinator will serve as a subject matter expert on MLOA and support Associate Director in all aspects of the pre-MLOA and MLOA return processes. Additionally, this position will assist with outreach and education for the campus community (students, staff, and faculty) regarding identification and intervention for students of concern and assist in the development of a network of community providers to supplement on campus services. 

About the Work Unit:
Student Affairs Students fosters wellbeing and success by identifying concerns and coordinating support for students facing life events that may interfere with their academic and personal goals using a proactive, collaborative, and student-centered approach. Student Care Coordination is the central and first point of contact for students to help identify needs and determine the most appropriate resources in Vanderbilt’s Student Care Network and in the Nashville community to address concerns. The CARE Support and Intervention Team within Student Care Coordination provide crisis intervention, care management, and support for students with elevated concerns, serious mental health challenges, students who pose a potential threat to themselves and/or others in the community, and students with other high acuity needs. This team will work closely with the Director of Student Care Network and Student Care Coordination and Assistant Dean for Community Standards and Student Support, along with members of the CARE team, Welfare Panel, and other campus partners to help maintain the safety and health of Vanderbilt students. SCC offers a flexible work environment that embraces a hybrid approach, allowing a balance of working remotely and collaborating in the office with colleagues.

The Student Care Network (SCN) is comprised of the Center for Student Wellbeing, Student Care Coordination, the University Counseling Center, and the Student Health Center. The SCN is a holistic network of services and resources pertaining to health and wellness available to all Vanderbilt University students who have paid the Student Health Fee. As such, staff within the offices comprising the SCN work collaboratively to create integrated processes, referrals, and treatment plans to better serve the entire student.

Key Functions and Expected Performance:

CARE Support and Intervention Coordination:

Meet individually with students considering an MLOA to explore all available options. Ensure students understand the process to take an MLOA, including treatment expectations during an MLOA, identification of treatment providers and other support needs while on MLOA, exploration of safety and/or crisis planning that may be needed while on leave, and the process for return. Assist Associate Director with review process for return from Medical Leaves of Absence(MLOA), to include collection and tracking of all MLOA documentation, communication with students to answer questions and ensure documentation is complete, and uploading documentation to Maxient. Collaborate with appropriate campus partners (e.g. University Counseling Center, Center for Student Wellbeing, Student Health Center, Student Access, and academic partners) to ensure student needs can be met upon return. Complete return meetings, as appropriate, for students returning from MLOA and communicate outcomes with Associate Director to help facilitate return decisions. Maintain a small caseload of students returning from MLOA and provide ongoing support for at least one semester following their return. Assists Director and Associate Director with data collection and analysis related to MLOA and students of concern. Provide support for the Campus Assessment, Response, and Evaluation (CARE) Team and work closely with key stakeholders of the CARE Team and Welfare Panel to facilitate coordinated and holistic support measures for students of concern. Assists Associate Director in the preparation of relevant reports for weekly meetings related to students of concern so that information is distributed accurately, and campus partners are well- informed. At appropriate intervals throughout the semester, leads efforts to identify students who receive academic actions (i.e. midterm deficiencies, dismissals, probation, etc.), outreach to students to offer support, maintain updated documentation in student files, and notify appropriate campus partners. Monitor incoming Student Care Coordination intake forms for acute concerns, facilitate immediate outreach to assess safety, if necessary, and triage to CARE support team care coordinator for follow up. Meet individually with students of concern or who may have acute needs (and their families, if necessary) to coordinate and provide immediate service/resource connection. Be knowledgeable about and connect students with appropriate support services on and off- campus to help students effectively navigate the Student Care Network. Assists with incoming referrals from campus partners, including but not limited to the Student Health Center, University Counseling Center, and Student Access, to ensure students are appropriately connected to Student Care Coordination services. Provide back up support for administrative coordinator, as needed, to include answering main phone lines, processing intake forms, and completing payments for Student Care Assistance Program. Provide services to a diverse campus community to support campus and individual mental health, wellness, and skill development. Utilize electronic documentation systems to create and/or update wellbeing files in Maxient to share information with campus partners and guarantee accurate file maintenance.

Outreach and Development:

Build relationships with and maintain a network of community providers and resources that meet the needs (medical, financial, and otherwise) of the student population so that off-campus referral scan be made intentionally and successfully. Manage community relationships and update the Community Provider database which enables CARE Support Team and SCC staff to make accurate and efficient off-campus referrals. Engage in professional development to remain abreast of trends in higher education, student mental health, substance use, and wellbeing to ensure our practices are both relevant and evidenced-based. Assumes other responsibilities as directed by the Associate Director and/or Director in matters related to student wellbeing.

Other Duties:

Serve on divisional committees and working groups to enrich the experience and resources available to staff across the division. Participate in division wide events as needed (Move-In Day, Rites of Spring, Diverse ‘Dores Day, MLK Day, etc.) to support colleagues and ensure these events run smoothly. Maintain an understanding of institutional and departmental financial policies related to areas such as travel, procurement, expense, and time reporting to ensure compliance and proper stewardship of university resources. Demonstrate a willingness to support colleagues across the division in order to encourage the implementation or fulfillment of divisional goals and strategic priorities.

 

Supervisory Relationships:
This position does not have supervisory responsibility; this position reports administratively and functionally to the Associate Director of CARE Support and Intervention.

 

Education and Certifications:

A bachelor’s degree, with a concentration related to mental health and/or social work, is necessary. A master’s degree in social work, counseling, or related mental health field is preferred. License or eligibility to obtain relevant license (LMSW, LCSW, NCC, LPC, LMFT) or case management certification is preferred.

 

Experience and Skills:

One to two years of experience or the equivalent is necessary. Care management, intake/assessment, de-escalation and/or counseling techniques. Demonstrated skill in written and oral communication is necessary. Strong organizational, critical thinking, and problem-solving skills are necessary. Experience entering and tracking data is necessary. Specific knowledge, experience, and skills in providing services to a diverse student population. Evident language fluency in Mandarin, Cantonese, Korean, Japanese, Hindi, Bengali or other Asian languages is preferred. Ability to work effectively with students from diverse backgrounds attending highly selective institutions with a broad range of campus groups, departments, and organizations is preferred. Experience working in a University setting is preferred.
