We are seeking a Care Managment Coordinator to join our team on a Casual basis. The Care Management Coordinator is a member of an interdisciplinary team accountable and responsible for coordinating the care and service of selected patients and for the promotion of collaborative practice and continuity of care. As a result, effective utilization of resources and the achievement of desired clinical and financial outcomes will be accomplished. Will monitor the appropriateness of hospital admissions and extended hospital stays.

 

MINIMUM QUALIFICATIONS:

RN with an Associate's degree from an accredited College. Minimum five years’ experience as a Registered Nurse in a clinical setting, performing utilization review, case management, care planning, and quality improvement Current State of Wisconsin Registered Nursing license

 

Preferred Qualifications:

RN with a Bachelor’s degree from an accredited College or University.
