Job Overview:
The Caregiver Counselor administers individual counseling to Caregivers who meet the specified criteria under the Title III E Caregiver Grant. Facilitate the monthly Caregiver Support Group.

Required:
LCSW with at least 1 year experience in health care or LSW under the supervision of an LCSWScheduling Requirements:

Assesses and evaluates each caregiver's personal history and that of the person being cared forCompletes all system mandatory modules for employeesIndividualizes a care plan based on evaluation of all aspects of a client's current needs for support and referral to appropriate agencies for assistancePromotes program services to all health care providers in Ocean CountyProperly records all elements of visit on client's chartResearch any new resources for caregivers in Ocean County,Schedules appointments at convenient times for the client, including time for phone session if unable to come to centerProvides psychoeducation on disease process and behavioral changesCounseling to develop and enhance coping skills, communication techniques, and advocacy skillsCounseling for care planning and decision-makingFacilitates weekly Caregivers Support Group,Appropriately records all information on clients seen for monthly/quarterly grant reportsPerforms other duties as required by the departmentChoosing RWJBarnabas Health!
