As a Caregiver II, you’ll provide personalized, tailored care based on each client’s unique needs and care plan. External Job Description Provide personal caregiving services in accordance with an established plan of direct care.Provide for the personal needs and comfort of people in their own homes.Assist patients with activities of daily living.Perform general housekeeping activities.Provide transportation assistance to and from activities and appointments.Offer compassionate companionship to clients.Serves as the care plan liaison between the client served and the branch. Actively supports and represents the company's private pay initiatives including but not limited:  Taking an active part in growing referral sources when opportunities arise, working knowledge of marketing resources, recruitment, referrals, etc.  Assist company’s goal of being a primary agency of choice of homecare private pay clientele.Other duties as assigned. Qualifications HS Diploma/GED Preferred required, additional professional certifications or associate degree is a plus1+ years of accumulated experience in HomeCare/Long-Term Care setting. (Internal Candidates must have a minimum 6 months employed with AWC)Minimum of at least one letter of recommendation Dependable/Flexible/Available on evenings and weekendsBilingual preferred but not requiredValid Driver’s License in good standing required from current state of residencyMeet company standards for an approved driverMust pass company background check and applicable State required background checksStrong ability to communicate both verbally and writtenCapable of working responsibly with confidential informationMust be able to work independently or collaborative environmentMust demonstrate strong interpersonal and communication skillsAccountable and goal-driven professional who takes pride in providing positive care and customer serviceExtended Continued Education and willingness to actively take part in ongoing education/trainingAbility to travel between clients as neededMust have Reliable Transportation/Auto Insurance About our Line of Business
