Overview
Genesee Lake School, a MyPath company, is a nationally recognized employee-owned provider of educational, vocational, and therapeutic residential services. These programs support adolescents, teenagers and young adults with Autism Spectrum Disorders, anxiety and mood disorders, and complex trauma. As a Case Coordinator you will:
Prepare, develop, and review the student’s overall treatment and support plan, in conjunction with the therapeutic support specialist.Completes assessments of the student, including the Mental Health Assessment and the Risk Assessment. Analyzes progress and data to support continued appropriateness/relevance of the plan.Establish treatment goals and support strategies and monitors the implementation of treatment goals and strategies.Schedule, coordinate, facilitate, and participate in meetings with the care team.Serve as the central communicator with members of the care team and families to address needs for specialized individual support. Provides timely and professional communication to family members and funding sources.Promote positive, proactive discharge planning throughout the student’s stay.Identify needs and long term plans including daily living skills, reunification, and discharge resources for students and their family.Ensure services are provided and documented in accordance with COA standards, funding sources, licensing agency requirements, and company policies/procedures.Serve as a training resource during new staff orientation by providing classroom instruction.Qualifications:
Bachelor’s or Master’s degree in Social Work, Psychology, or other related field required.Previous case management experience preferred.Demonstrated knowledge, skills, and experience working with children with Autism and Pervasive Developmental Disorders strongly preferred.Ability to be flexible and adaptable, critical thinking, highly organized, and excellent customer service skills.Excellent written and verbal communication skills required.Demonstrates an ability to exercise good judgment and effectively solve problems.Remains calm under pressure and maintains self-composure/self-controlEffective working alone and as a member of a team.Satisfactory attendance and performance record.Schedule & Benefit Information
