This position is responsible for ensuring high quality customer service, day to day operations, daily productivity monitoring and the financial effectiveness of the remote program.
How you Will make a difference
+ Receive phone calls and field basic questions; escalate critical issues to appropriate staff for resolution.
+ Update Managed Care grids to intranet and communicate changes to the appropriate centers timely.
+ Establish and maintain regular communication with the field through effective channels, including development of a Care Management newsletter.
+ Assist with the creation and deployment of regular educational offerings to the facility/center-based Care Managers.
+ Collaborate on a regular basis with Managed Health Services on any outstanding contract updates needed.
+ Run daily reports in order to be used by Care Management Support team to proactively identify opportunities in facilities/centers; assist with follow up efforts based on reported data as needed.
+ Support the Transitional Care Program as requested, to include follow up phone calls and arranging follow up primary care physician appointments.
What you Need to make a Difference
+ Associates degree required, but Bachelor’s degree preferred.
+ Current licensure as a Registered Nurse in state employed and have a multi-state license (Compact), preferred.
+ Minimum of three (3) years related experience.
+ Knowledge of billing procedures for clinical and social services.
+ Knowledge of clinical and social services documentation procedures and standards.
+ Knowledge of community health services and social services support agencies and networks.
+ Knowledge of state and federal regulatory requirements.
The ideal candidate would have a strong background in supporting clinical process and work flows. Remote Opportunity, if located in Louisville, KY the option to work hybrid or full time in office is available.
