This is a Full-time position. SUMMARY: Serves as a key team member of the UR/Case Management department.  Plans, coordinates and provides patient education, discharge planning and post discharge follow up. Performs utilization review functions including documentation review for admission criteria per MCG guidelines, physician advisory, and provision of clinical information for insurance authorization. Conduct audits and assist with denials in reimbursement. Coordinate Swing Bed patient care including referrals, comprehensive assessments, interdisciplinary group meetings and discharge planning. Provide community education as needed. Alternates with team members to provide weekend case management coverage.    EDUCATIONAL/EXPERIENCE REQUIREMENTS:  Registered Nurse with minimum of 2 years nursing experience with at least one of these years occurring in the last 12 months. Preferred clinical experience in case management or utilization review.    REQUIREMENTS Current CPR    LICENSURE / CERTIFICATION REQUIREMENTS: Current Texas RN license or RN compact licensure.   OTHER REQUIREMENTS: Must be able to communicate effectively with patients, caregivers, physicians and other members of the interdisciplinary team.  Bi-lingual preferred but not required.
