Job Summary: Provide formal assessment to children 0-18 and adults 18 years and older to determine eligibility for the CAP Medicaid program. Regularly monitor all home and community based services being utilized by the CAP participates.
Minimum Requirements: RN with current North Carolina License and at least three years in an acute care setting or BSW or other related human service field with three years experience in a geriatric social work setting. Graduate from an accredited School of Nursing. Have and maintain a valid license to operate a motor vehicle in the State of North Carolina. Typing, basic computer knowledge, general knowledge of office equipment required. Current CPR certification required.
