AltaPointe operates several programs that meet the wide variety of needs for the newly homeless and chronically homeless. Most patients are in active psychiatric treatment, while a few have lost contact with their treatment teams due to their homeless status. Some patients need a higher level of psychiatric and community resources than others. Some patients live alone, some live in small groups. Some residences are single-family dwellings while others are apartments. All residents and residences are diligently monitored by AltaPointe professionals to guarantee the high standards AltaPointe demands. The objective of all our homeless initiatives is to link patients with community resources in order to build the skills necessary to eventually live productive, independent lives.
ResponsibilitiesClinical Responsibilities:
Demonstrates the ability to assess the consumer’s needsIdentifies goals and plans for appropriate services utilizing the ISPProvides appropriate linkage to community resourcesProvides services to maintain placement in the least restrictive environmentProvides ongoing monitoring of the consumer’s progress toward goalsAdvocates on behalf of the consumer to secure community resourcesEffectively collaborates with the treatment team members and outside sources in the deliverance of case management servicesMakes multiple contacts to consumers per week as neededAssist consumers with accessing AltaPointe Health Systems Intake and follow up appointments.Assists consumers with transportation needs through linkage and/or teaching methodsTransports consumers when appropriate and necessary.Teaches basic living skills to improve the consumer’s quality of lifeCollaborates with the staff at 15 Place to ensure that wrap around services are present, as necessary to develop a plan to end consumer’s current homelessness episodeAssist with CCN referrals as needed and provided by Housing FirstMakes referrals to housing programs both within the agency and outsideContinues case management with individuals who are placed as needed to provide linkage to needed servicesAttends various area breakfasts, at least twice weekly, in the morning to build rapport with the members of the target populationWill go to at least two area shelter and/or camp in the afternoon three times per week.Will document contacts in HMIS program as per grant requirementsWill document contacts in both HMIS and Avatar to manage consumer contacts

Case Load Management
Effectively manages caseload based on consumer needsEnsures that case load of PATH eligible consumers are accurate in AVATAR QualificationsBachelor’s Degree in a mental health discipline
Must be 23 years of age or older, has and maintains a valid Alabama driver’s license, and must have proof of current liability insurance.
