The overall goal of the position is to enhance the quality of patient management and satisfaction, to promote continuity of care and cost effectiveness through the integrating and functions of case management, utilization review and discharge planning. The Case Manger must be highly organized professional with great attention to detail adaptable to frequent change, and compliant with regulatory and departmental guidelines and policies. **Responsibilities:** Patients/Families, Hospital staff, Medical staff, Community agencies, and Insurance Companies are served by conducting interpersonal relationships in a manner that projects a positive image of the department and hospital.
